 COMPLETING THE FORM

Please complete all fields below for each parent and we will use that information for all of your children registered at YGW. Accuracy of the information provided is critical because it will be inputted into the database. Do not leave any data fields blank. If a field does not apply (eg, no cell phone or no work email, or if one is a single or separated parent), then enter “none”. All radio buttons for YES or NO selection are required. The check boxes are optional to further customize the contact methods for the different announcement categories.

All parent entries must be completed by November 10, 2017 to be included in the system verification test.

HOW THE ALERT SYSTEM IS USED

YGW sends out broadcast communications to the parent body for all types of reasons which fall into four categories:

1)     General Announcement – academic and administrative matters

2)     Priority Announcement – General Announcements with high priority or short deadline

3)     School Closure/Schedule Change – due to inclement weather, facility issues or late schedule changes

4)     Emergency Alert – notification of a true emergency and any actions or non-actions necessary

The available notification methods for each category follow. Parents may choose any and all methods available. Required methods for certain categories are noted.
